Grant Subrecipient Monitoring and Reporting Checklist
Organize reporting dates, source documents, review assignments, and follow-up records your agency determines are relevant to a particular grant program.

Start with the agency's actual award requirements
- Identify the program, agreement, funding period, authorized owner, and applicable review requirements.
- Record each subrecipient's approved scope, designated contacts, and agency-required documents.
- Separate confirmed award conditions from assumptions, incomplete records, and unresolved questions.
Build a reviewable reporting calendar
- List the reporting deliverable, due date, responsible agency reviewer, and current review status.
- Note which source document establishes the deadline and where submitted evidence is retained.
- Document missing items, agency-approved extensions, and the next staff-owned follow-up action.
Keep evidence connected to the agency decision
- Record what was reviewed, when it was reviewed, and who made the agency's determination.
- Identify missing documentation without treating an incomplete record as proof of noncompliance.
- Keep funding, payment, corrective-action, and external-communication decisions with authorized staff.
Define one realistic proposed pilot scope
Ask which single program needs review, which records the agency can authorize, and which reporting format staff would actually use. Any data access, workflow, reminder, integration, recordkeeping control, or production feature requires separate approval, implementation, and verification.
